Introduction

Pharmacodynamics & System Mechanics

Neostigmine drives intense accumulation of Acetylcholine (ACh) across two distinct receptor populations:

1. Nicotinic Motor Effects (NM)

2. Parasympathetic Off-Target Effects (M2,M3)

Because Neostigmine increases ACh everywhere in the peripheral nervous system, it triggers intense unwanted parasympathetic over-activation:

Target System Muscarinic Effect Systemic Manifestation
Cardiovascular (M2) SA node suppression Severe Bradycardia / AV block
Respiratory (M3) Bronchoconstriction & hypersecretion Bronchospasm & airway flooding
Gastrointestinal (M3) Increased motility & secretion Profuse salivation, nausea, diarrhea
Ocular (M3) Pupillary sphincter contraction Miosis

The Antimuscarinic Co-Administration Mandate

Relevance (Dental & Maxillofacial Context)

1. Post-Surgical Neuromuscular Blockade Reversal

2. Myasthenia Gravis Management
